Dive doctor — the 24 h line Prefilled with the hotline DAN publishes for that country.

Your own emergency number The number the hotline rings back on. Whoever answers it must know this plan. Nearest hospital Have you phoned the hospital yourself? Recompression chamber Leave this empty unless you have phoned it yourself and someone answered. Have you phoned the chamber yourself? Oxygen on site What there is, where it is kept, and how long it runs. Getting off the water, leg by leg One leg per line. They print numbered, in your words and your order.

Who keeps this plan current Date you last checked the numbers

Reprint the sheet when it is more than 180 days old — a scanning phone warns the diver after that, and so it should. Phone the hospital and the chamber yourself before you write them down: a plausible wrong number at two in the morning is worse than a blank line.
